Accident Investigation & Preparedness

Is your organization truly prepared for its next serious incident or accident? How should you prepare? What should you expect after an accident during the onsite investigation, follow-up engine testing, wreckage analysis, the months that follow, or depositions that may surface? Line Operations Safety Audits (LOSA)

A Line Operation Safety Audit (LOSA) provides the data to show how pilots perform during normal daily flight operations. When first learning about LOSA, people often ask how it differs from other voluntary safety tools such as ASAP, ASRS, FOQA, IEP, or VDRP. While each of those is a valuable tool in a robust safety management system, they leave gaps in the complete picture. Some of those programs provide the pilot’s perspective and others the aircraft’s perspective but only LOSA provides a third-party perspective while also including the environment at the time of the observation. LOSA is a relatively new tool in the helicopter industry and it has been embraced by operators in the oil & gas and air ambulance sectors.
